How to register
We register all patients that live within our catchment area as shown on the map below.
Patients within the catchment area will have access to our full range of services, including home visits and liaising with community services local to them where needed.
Patients who live outside of our catchment area can register with us (or remain registered with us if they move) on the understanding that we will not be able to provide home visits and they will need to attend us in person if face to face care is needed. Where a home visit is required, they can contact 111 who will be able to help.
We reserve the right to remove patients from our list if we feel the care we can provide is compromised by their need for a home visit that we cannot provide, their difficulty in getting to us to receive face to face care, or our ability to work with community services local to them that we do not normally work with.
Catchment Area
You may choose to see any of our doctors, and we will try to offer appointments with the doctor of your choice, timetable and holidays permitting. All doctors have access to your computerised medical record.
If you have been registered with a GP practice in the UK before, please provide your NHS number.
If you have, please provide:
- One recent proof of address (e.g. a utility bill dated within the past three months)
- One proof of residency (e.g. passport, Home Office papers, a full UK drivers’ licence).

However, you may also come to practice and speak to our reception team if you are not able to complete it online.
