Some of the information you have supplied to us will be held on computer or on paper-based files.  They are your confidential medical records. 

All staff members at the surgery are bound by law to keep your records confidential. 

Sometimes we may share this information with other health professionals involved in your care to help them meet your needs. 

You can ask to see your own records, but we need a few days to arrange this and you will need to fill out a Subject Access Request (SAR). Please ask at Reception.
